ALS in the Heartland's Patient Financial Request Program
Our Financial Request Program may be able to provide up to $500 to help offset the cost of equipment, home modifications, or other essential expenses that can improve safety, independence, and quality of life for individuals living with ALS.
Examples of Assistance May Include:
- ALS-related needs
- Home modifications
- Mobility and adaptive equipment
- Other essential expenses that support safety, independence, and quality of life
Please note: We ask families to check with our Equipment Loan Program first for available equipment before submitting a request.
Eligibility & Funding:
All requests are reviewed individually. Financial assistance is provided on one occasion, based on eligibility and available funding.
